Actions help you track follow-up tasks and ensure safeguarding concerns are addressed promptly. Signal can also suggest relevant actions using AI.

What are Actions?

Actions are tasks that need to be completed in response to an incident or safeguarding concern. They help ensure nothing falls through the cracks and provide a clear audit trail of what was done. Common actions include:
  • Contact parents/carers
  • Speak to student
  • Monitor behavior
  • Refer to external agency
  • Update care plan
  • Arrange meeting
  • Complete assessment
  • Follow up with staff

Creating Actions

Open the Incident

Navigate to the incident that requires follow-up

Click Add Action

Select + Add Action in the Actions section

Fill in Details

Enter action description, assign to staff member, and set due date

Save

Click Create Action to save

Action Details

When creating an action, you can specify:
When the action was or will be taken. This helps track chronology of follow-up activities.
A clear, specific description of what needs to be done or was done.Good examples:
  • “Contact mother to discuss behavior concerns from today’s incident”
  • “Arrange meeting with SENCO to review support plan”
  • “Monitor student’s behavior in playground for next 3 days”
Poor examples:
  • “Follow up” (too vague)
  • “Sort it out” (unclear what action is needed)
If the action involves an external agency, select it from the dropdown. This helps track multi-agency involvement.
The staff member responsible for completing this action. The assigned person can track the action in their task list.

AI-Suggested Actions

Signal analyzes each incident and suggests relevant follow-up actions based on:
  • Incident content and severity
  • Configured safeguarding policies
  • Best practice guidance
  • Similar incident patterns

Using AI Suggestions

View Suggestions

After creating an incident, scroll to the Suggested Actions section

Review Each Suggestion

Signal will list 3-5 recommended actions with rationale

Select Actions to Create

Check the box next to actions you want to create

Customize if Needed

Click Edit on any suggestion to modify before creating

Assign and Create

Assign each action to a staff member and click Create Selected Actions
You can dismiss suggestions you don’t want to use. Signal learns from your choices to improve future suggestions.

Example AI Suggestions

For a bullying incident, Signal might suggest:

Immediate Action

“Contact parents of both students to inform them of the incident and actions taken”

Student Support

“Arrange check-in with student (victim) tomorrow to ensure they feel safe”

Investigation

“Speak to staff on duty at break time to gather additional context”

Monitoring

“Monitor both students’ interactions for next 2 weeks and record observations”
AI suggestions include policy references when applicable, helping you understand the guidance behind each recommendation.

Managing Actions

Action Dashboard

View and manage all your actions from the Actions dashboard:
Actions assigned to you, organized by:
  • Overdue: Past due date (red indicator)
  • Due Soon: Due in next 3 days (yellow indicator)
  • Upcoming: Due in 4+ days
  • Completed: Already finished

Completing Actions

Open the Action

Click on any action to view details

Mark Complete

Click Mark as Complete to record the action as finished
If additional follow-up is needed, create a new action from the incident.

Editing Actions

You can edit action details if circumstances change:
  • Update the action date
  • Modify the description to add clarity
  • Change the assigned person
  • Update agency involvement

Action Notifications

Stay on top of actions with automatic notifications:
Receive emails when:
  • An action is assigned to you
  • An action is due tomorrow
  • An action becomes overdue
  • An action you created is completed
Configure notification preferences in your User Settings.
See notifications in Signal when:
  • New actions are assigned
  • Actions are approaching due date
  • Actions are reassigned to you
  • Comments are added to your actions
Check the notification bell icon in the top right.
Optional daily email summary showing:
  • Actions due today
  • Overdue actions
  • Recently completed actions
  • Team action overview (for managers)
Enable in User Settings > Notifications > Daily Digest.

Best Practices

Be Specific

Write clear, actionable descriptions so anyone can understand what needs to be done

Assign Appropriately

Assign actions to the staff member best placed to complete them

Track Agency Involvement

Record which external agencies are involved for multi-agency oversight

Complete Promptly

Mark actions complete as soon as they’re finished

Troubleshooting

  • Check your notification settings in User Settings > Notifications
  • Verify your email address is correct in your profile
  • Check your spam/junk folder
  • Ensure notifications are enabled for Signal in your email client
  • Check if it’s in the Completed tab
  • Use the search function (search by description or assignee)
  • Check filters—you may be filtering it out
  • Verify you have permission to view that incident
  • Check you have permission to complete this action
  • Verify you’re the assigned person (or have appropriate permissions)
  • Check that relevant policies are configured
  • Ensure the incident description contains enough detail
  • Provide feedback on suggestions to improve future recommendations
  • You can always create custom actions instead
