Actions are tasks that need to be completed in response to an incident or safeguarding concern. They help ensure nothing falls through the cracks and provide a clear audit trail of what was done. Common actions include:- Contact parents/carers
- Speak to student
- Monitor behavior
- Refer to external agency
- Update care plan
- Arrange meeting
- Complete assessment
- Follow up with staff

When creating an action, you can specify:Date & Time

When the action was or will be taken. This helps track chronology of follow-up activities.

A clear, specific description of what needs to be done or was done.Good examples:
- “Contact mother to discuss behavior concerns from today’s incident”
- “Arrange meeting with SENCO to review support plan”
- “Monitor student’s behavior in playground for next 3 days”
- “Follow up” (too vague)
- “Sort it out” (unclear what action is needed)
Agency Involved
Agency Involved
If the action involves an external agency, select it from the dropdown. This helps track multi-agency involvement.
Assigned To
Assigned To
The staff member responsible for completing this action. The assigned person can track the action in their task list.
AI-Suggested Actions
Signal analyzes each incident and suggests relevant follow-up actions based on:- Incident content and severity
- Configured safeguarding policies
- Best practice guidance
- Similar incident patterns
Using AI Suggestions
You can dismiss suggestions you don’t want to use. Signal learns from your choices to improve future suggestions.
